Abstract

Asthma is described as uncontrolled and severe or refractory, which despite high doses of inhaled corticosteroids plus a long-acting beta2 agonist over the previous year remains uncontrolled, or which has required oral corticosteroids over at least 6 months of the previous year. The first step with these patients is a systematic assessment to confirm the diagnosis of asthma, discount therapeutic non-adherence or incorrect inhaler technique, and to detect and prevent aggravating conditions. Once these objectives have been realised, treatment will depend on the endotype. For patients with Th-2 profiles, we have the monoclonal antibodies anti-IgE (omalizumab) or anti-IL5 (mepolizumab, reslizumab), and treatment with macrolides for non-Th-2 patients. If poor control persists despite these treatments, we will consider depot corticosteroids or bronchoscopic techniques.
