Abstract

The principal feature of delirium is disordered attention and level of consciousness of acute onset and fluctuating, usually accompanied by other cognitive changes. It is usually transitory and of short duration. It is common in older people, especially if they have dementia. Its aetiology is multifactorial and based on a predisposed substrate (identifiable risk factors). It is potentially reversible with appropriate comprehensive geriatric management. It is considered a medical emergency and requires a systematic preventative and therapeutic approach. Thirty to forty percent of episodes can be prevented by multi-component, non-pharmacological programmes that act on modifiable risk factors, and strategies that form the basis of treatment once an episode of delirium has occurred. Symptomatic drug treatments (principally neuroleptic treatment), whether preventive or therapeutic, have shown no clear efficacy and are not currently recommended, except for very specific cases.
