Resumen

Los trastornos de ansiedad son los trastornos mentales más frecuentes en nuestro entorno sociocultural y con mucha frecuencia se consultan en Atención Primaria. Son enfermedades tratables y de larga evolución. En este trabajo se aborda el tratamiento de los trastornos de ansiedad más frecuentes en la práctica clínica diaria: el trastorno de ansiedad generalizada, el trastorno por crisis de angustia o pánico, la fobia específica y la fobia social. Se hace una revisión de las guías clínicas más destacadas y de metaanálisis que tratan de ordenar la gran disparidad de tratamientos disponibles para estas patologías. Además de una revisión general de las psicoterapias y de los psicofármacos que han demostrado evidencia en el tratamiento eficaz, se proporciona información del manejo de ambos tipos de tratamientos en cada uno de los diagnósticos referidos. En general, se puede decir que los antidepresivos y más concretamente los inhibidores selectivos de recaptación de serotonina (ISRS) se han convertido, desde el punto de vista farmacológico, en la herramienta terapéutica más recomendada para los trastornos de ansiedad. Sin embargo, y a pesar de haber sido señaladas como cuestionables, las benzodiacepinas siguen siendo utilizadas por los clínicos en todas las variedades de ansiedad pero muy especialmente en la ansiedad aguda. De las psicoterapias disponibles, la terapia cognitivo conductual es la que ha demostrado en distintos ensayos y en sus distintas variantes más eficacia. Se hace imprescindible la implantación de estas estrategias terapéuticas para la ansiedad en el ámbito de la Atención Primaria.

Abstract

Anxiety disorders are the most frequent mental disorders in our sociocultural setting and are very frequently consulted for in Primary Care. They are treatable and long course diseases. This work approaches the treatment of the most frequent anxiety disorders in the daily clinical practice: generalized anxiety disorder, anxiety or panic attack disorder, specific phobia and social phobia. A review is made of the most outstanding Clinical Guidelines and metaanalyses that attempt to put the great disparity regarding the treatments available for these conditions into order. In addition to a general review of the psychotherapies and psychopharmaceuticals that have demonstrated evidence in the effective treatment, information is provided on the management of both types of treatment in each one of the diagnoses reported. In general, and from the pharmacological point of view, it can be said that the antidepressants and more specifically the SSRIs have become the most recommended therapeutical tool for anxiety disorders. However, and in spite of being indicated as questionable, benzodiazepines continue to be used by the clinicians in all varieties of anxiety, but very especially in acute anxiety. Of the available psychotherapies, cognitive behavioral therapy is that which has demonstrated in different trials and its different variants to be the most effective. Implementation of these therapeutic strategies for anxiety within the Primary Care setting is essential.
